Introduction
The United States healthcare system has been a source of great concern to policy-makers, healthcare providers, and patients alike. Despite being one of the wealthiest nations in the world, the U.S. has not been able to provide its citizens with universal access to quality healthcare services. The current healthcare system is plagued with issues such as high costs, unequal access to care, and fragmented care delivery. Therefore, designing an ideal healthcare system is necessary to ensure that every American has access to quality healthcare services.
Overview of Ideal Health Care System
My ideal healthcare system would be universal, accessible, affordable, and of high quality. The system would be designed to provide comprehensive care to patients, regardless of their income or health status. It would be based on the principles of patient-centered care and focus on prevention and wellness.
The ideal healthcare system would prioritize preventive care measures such as regular check-ups, vaccinations, and cancer screenings. The system would be equipped with state-of-the-art technology, electronic health records, and telemedicine to improve healthcare delivery and efficiency. The healthcare workforce would be well-trained, adequately compensated, and provide evidence-based care.
Recent Health Care Reform Legislation and Progress
The recent healthcare reform legislation, such as the Affordable Care Act (ACA), has been instrumental in making progress toward achieving an ideal healthcare delivery system. The ACA has enabled millions of Americans to gain access to health insurance coverage, especially those with pre-existing conditions, through its Medicaid expansion and health insurance exchanges. The ACA has also promoted preventive care services, such as mammograms and colonoscopies, without any out-of-pocket costs for the patients (Garber et al., 2014).
However, the ACA has also faced significant opposition from some quarters, particularly from those who see it as a government overreach. Additionally, while the ACA has helped to increase access to care, it has not significantly addressed the underlying issues of high healthcare costs and fragmented care delivery.
Elements of the Current Health Care System to Keep and Change
Primary care is an element to keep in the ideal healthcare system. It is the foundation of any healthcare system, and the current system recognizes this. Primary care providers act as the gateway to specialty care and are often the first line of defence in preventing and treating chronic illnesses. Retaining a focus on primary care would help to ensure that patients receive comprehensive, coordinated, and cost-effective care.
Evidence-based medicine is a vital element in an ideal healthcare system. Evidence-based medicine is the use of research and clinical expertise to inform clinical decision-making. It is an essential element of quality healthcare, and the current system has made strides in promoting its use (Wendt & Bambra, 2020). Retaining a focus on evidence-based medicine would help to ensure that patients receive the most effective and appropriate treatments.
Change is the initiation of progress and success; therefore, one of the most significant changes would be the implementation of a universal healthcare system. The current system leaves millions of Americans uninsured, and even those with insurance often face high out-of-pocket costs (Garber et al., 2014). A universal healthcare system would ensure that all Americans have access to affordable, high-quality healthcare.
Value-based care would be a crucial component in the healthcare system. The current system is largely fee-for-service, meaning that healthcare providers are paid based on the volume of services they provide rather than the quality of care they deliver. Value-based care, on the other hand, rewards providers for delivering high-quality, cost-effective care. This would help to reduce healthcare costs while improving health outcomes (Wendt & Bambra, 2020). In summary, the ideal U.S. healthcare system would retain a focus on primary care and evidence-based medicine while also implementing universal healthcare and value-based care. These changes would help to ensure that all Americans have access to high-quality, affordable healthcare.
Paying for Health Care
In an ideal system, healthcare would be paid for through a combination of public and private funding sources. The government would play a role in financing universal access to care, while individuals would contribute through premiums, co-pays, and deductibles. The system would also promote transparency and competition to help control the costs.
Innovative Ideas to Improve Health Care System
The U.S. healthcare system is in need of significant innovation to improve access to care, reduce costs, and improve patient outcomes. Here are a few innovative ideas that could help to transform the system:
One potential innovation is telehealth, which is a technology that allows patients to access healthcare services remotely, using videoconferencing, phone, or other digital technologies (Simmons et al., 2016). This could help to improve access to care, particularly in rural or underserved areas where there may be a shortage of healthcare providers. By allowing patients to access care from the comfort of their own homes, telehealth could also help to reduce the spread of contagious diseases (Garber et al., 2014). Electronic Health Records (EHRs) allow medical practitioners to efficiently and securely access and share patient information. This could help to reduce medical errors, improve care coordination, and reduce healthcare costs by eliminating the need for paper records.
Artificial intelligence (A.I.) is another potential innovation that could transform many aspects of healthcare. For example, A.I. could help to identify patients at high risk of developing chronic illnesses or help to identify the most effective treatments for a given condition (Simmons et al., 2016). By using machine learning to analyse large datasets, A.I. could also help to identify trends and patterns that could lead to better health outcomes for patients.
Finally, health information exchanges (HIEs) could improve the U.S. healthcare system. HIEs are essential for ensuring the secure and efficient sharing of patient information. This could help to improve care coordination and reduce medical errors, as well as reduce healthcare costs by eliminating the need for duplicate tests and procedures (Habicht et al., 2023). By enabling healthcare providers to share information across different systems and organizations, HIEs could also help to improve population health management with no limitation to other technological innovations such as cancer immunotherapy, digital therapeutics, integration of data and predictive analysis and other medical innovations.
Ideal Programs and Incentives
One possible new program could be the creation of a national public option health insurance program, which would compete with private insurance companies and provide an affordable alternative for individuals and small businesses. This program could be structured to negotiate lower prices with healthcare providers and pharmaceutical companies, reducing overall healthcare costs. In terms of incentives, one approach could be to provide financial rewards for healthcare providers and institutions that improve health outcomes and reduce healthcare costs. This could be achieved through pay-for-performance programs that reward providers for achieving specific quality measures and cost savings targets (Habicht et al., 2023). Another incentive could be to offer loan forgiveness programs for healthcare professionals who choose to work in underserved areas or specialize in primary care, which would help to address shortages in these areas.

In summary, an ideal U.S. healthcare system would prioritize universal access to affordable and high-quality care, focusing on preventive care and early intervention. Recent healthcare reform legislation has made progress toward achieving this ideal, but there is still room for improvement. We should keep and improve elements of the current system while changing those that are inefficient or inequitable in health.
